White shirt (t-shirt)
Red sash for the waist
Eye patch (more so for the boys)
Grey ripped skirt (Girls)
Grey pants or shorts (Boys)
Black leggings (Girls)
Plastic sword

Instructions:
You can buy all of these or the complete costume by itself, then you can just put it together.

Other information to consider:
You can also use face makeup if you want to draw any scars, or make them have black teeth, if you know what I mean :)
