1 reg. size white cotton pillow case.
Scissors.
Black marker.
Anything to use to trace a 5" circle. (i.e., 5lb coffee can lid, etc.)

Instructions:
Lay the pillow case flat, cut a large circle (about 5" round)so that your childs entire face is visible. I used a 5lb coffee can and traced around it. The circle should be centered, and the top of the circle should be about 3" down from the sewn edge of the pillow case. About 10" down, use the black marker to draw "Pillow" across the chest. Cut out arm holes large enough not to restrict arm movement (about 4" or 5" in diameter. Slip the pillow case over your child's head and away you go! Make adjustments if needed.

Other information to consider:
When my son was 2, I was sooo busy that year that I forgot all about Halloween, so in a pinch, I made this costume up in about 10 minutes. Everybody loved it! You could use a different colored pillow case, but the white one worked beautifully.
