Items you will need: (Hit your return key to start a new line)
cardboard box
yellow posterboard
blue, red, white, and black poster paints
black headband
black felt
8-10 white bone things (or stencils, whatever)
black elastic fabric (should be long enough to be stapled on both ends of box and rest on child's shoulders)
red face paint
3 plastic bags
3 stuffed dogs that fit in the bags
black clothes to wear underneath
stapler or staple gun
glue

Instructions:
Take posterboard and paint a red and blue stripe around the edges. Paint "puppies for sale" on the middle. Glue one bone stencil on posterboard. Paint black outline around. Glue posterboard on cardboard box. Staple the black elastic straps to the inside of the box. Staple plastic bags on box.

Take black felt, cut out two ear shaped things and paint middle white. Hot glue onto headband.

Take a paper bag and glue on the rest of the bone stencils. Outline in black paint. This can be used for candy.

Put on black turtleneck, pants, and box.

Place puppies in plastic bags.

Paint red tongue outside of child's mouth with red face paint.

Place headband on head and go trick or treating!

Other information to consider:
Instead of using stencils, you can paint the actual bones. You kinda have to be really artistic for that. Also, using black lipstick or something, you can blacken the nose and draw whiskers on the face. This costume is really cute!
