Instructions for using this technique:
Each teacher signs the sheet each day with comments about turned in homework, worked well in class, arrived on time and prepared with materials. Also, there is an area for comments about upcoming assignments. The parent has to sign and return sheet to child's counselor. These are kept on file.

Other information parents should consider before using this technique:
When your child is given a poor grade at the end of the marking period, you can refer back to the tracking sheet. If there are no marks on the tracking sheet indicating this type of grade was coming, you can ask the teacher why he failed to indicate this on the tracking sheet. It forces the teacher to be consistent, and also lets the child know they are being watched on a daily basis and have to stay on top of things. It really helps and has been a powerful tool for our ADD middleschooler.
