General purpose of letter:
This is a sample letter sent to a teacher when you need to request a parent/teacher conference. Make sure that you state your reasons for requesting a conference. Include your contact numbers as well as the times you can attend a meeting. As an example, this letter describes a child who has experienced a mood change and a lack of desire to go to school.

Dear Ms./Mrs./Mr. [Name of Teacher]:

I am concerned about a new development with [Child? Name], who has been returning home from school in a despondent mood. I would like to schedule an appointment for a conference with you to discuss this current situation.
