Sample letter:
DateMs./Mrs./Mr. [Name of Teacher] Job Title School Name School Address City, State, Zip Code Dear Ms./Mrs./Mr. [Name of Teacher]: I am concerned about a new development with [Child? Name], who has been returning home from school in a despondent mood. When I ask her/him what is wrong, she/he reports that she/he no longer wants to go to school because she/he hates it. This is quite a shock to me, as [Child? Name] has always loved school. I would like to schedule an appointment for a conference with you to discuss this current situation. Please call me at home, [home phone number), or at the office, [office phone number], at your earliest convenience. Thank you. Sincerely, [Signature] [Your Name] [Parent?/Guardian?
