General purpose of letter:
This is a sample letter sent to the teacher when your child has a disability. When writing your letter, identify the problem clearly to ensure that the teacher understands your childĘs needs and can accommodate your wishes. As an example, this letter addresses the needs of a child with a hearing disability.

Dear Ms./Mrs./Mr. [Name of Teacher]:

I want to thank you for our parent/teacher conference, as I was concerned about [Child? Name] behavior in class. As per our discussion, I took [Child? Name] to the doctor, and it turns out that she/he has lost hearing in her/his left ear. I am taking [Child? Name] to a specialist to assess our options. I will keep you informed on any new information about her/his condition. Based on this new discovery, it would be very helpful if [Child? Name] could be moved to the front of the class making it easier for her/him to hear your lessons.
