General purpose of letter:
This is a sample letter to the nurse if your child has asthma. Include an asthma kit and a letter from the doctor stating that the asthma is under control. List examples of what can bring on an attack. List all contact numbers including your doctorĘs number for the nurse to have on file.

Dear Ms./Mrs./Mr. [Name of Nurse]:

Over the summer my child, [Child? Name], began having asthma attacks. Enclosed is a note from our doctor indicating that [Child? Name] is capable of attending school. I am sending you an asthma kit, as [Child? Name] occasionally needs to use an inhaler. I have sent a note from the doctor to [Name of Gym Teacher], that explains the activities [Child? Name] needs to be excused from in gym.
