General purpose of letter:
This is a sample letter with instructions from you and your childÆs doctor to the school nurse indicating when to administer medication to your child. Make sure that the instructions are precise, and list all contact numbers including your doctorÆs number in case the nurse has any questions. As an example, this letter describes in detail what needs to be done if your child has an ear infection.

Name] to your office after lunch.
