General purpose of letter:
You can use this sample letter to inform the school nurse about any of your childÆs allergies or illnesses. It is especially helpful to include emergency kits that your child will need in case of an accident. Be specific in listing anything that can affect your childÆs health, positive or negative. List all contact numbers, including your doctorÆs number, for the nurse to have on file. As an example, this sample letter describes in detail what needs to be done if your child is allergic to peanuts.
Sample letter:

Enclosed is an emergency epinephrine auto-injector in case of an accident.
