General purpose of letter:
This is a sample letter requesting a conference with the guidance counselor/principal to discuss support services for your child. This conference would come after attending a parent/teacher conference where your childĘs special needs have been identified. Include all the pertinent information you have so that the counselor/principal can be prepared for a meeting with you. List your contact numbers and the times you can attend a meeting.

Dear Ms./Mrs./Mr. [Guidance counselor?/Principal? Name]:

[Teacher? Name], who is [Child? Name] [grade/subject] teacher, recommended that I make an appointment with you. [Child? Name] has developmental challenges that need to be addressed, and I would like to see what my options are for getting [Child? Name] the support she/he needs. I have enclosed [Teacher? Name] report for your review.
